DEATH CERTIFICATE

The information on this form allows us to complete the death certificate, in either Oregon or Washington.

EMBALMING AUTHORIZATION

This form allows us to proceed with the embalming process.  It should be signed by the person being embalmed, the next of kin, or person in charge of disposition.

CREMATION AUTHORIZATION

This form allows us to proceed with the cremation process.  It should be signed by the person being cremated, the next of kin, or person in charge of disposition.

DISPOSITION OF CREMATED REMAINS

This form instructs us on the what to do with the cremated remains after the cremation.
